I give my consent for my child(ren) to participate in the annual "Circle of Grace" program which will be taught during the 2023/2024 faith formation sessions in accordance to the AOD Safe Environments Office.

**If I do not consent I will type in "No Circle of Grace" in the 'Special Learning Needs" section of the registration form.**

St. Mary may use my child(ren)'s likeness and/or name on the website and/or other publications of the parish.

** If St. Mary cannot use my child(ren)'s likeness I will type in "No Photos" in the 'Special Learning Needs" section of the registration form.**

If I am signing up for At-Home Catechesis; Our family commits to:

- picking up the lesson packets from the Faith Formation Office when notified that they are ready

- completing the weekly lesson and online assessment by Saturday at Noon each week

- Taking part in family activities at church as scheduled

I agree to pay tuition in full within 30 days of classes starting in the fall. Tuition per family is $90 and an additional $25 if my child(ren) is enrolled in a sacramental prep (1st Communion or Confirmation) class.
